The Elder Care Coordinator

When you or an elder loved one needs help, needs usually go beyond legal questions. Often, the primary need is physical care — and answering questions concerning the right type of care and its affordability. The process of finding these answers can be very stressful to all family members involved. Most elder law firms are ill-equipped to find these answers or alleviate this stress.

However, answering these questions and alleviating this stress is exactly the value that our Elder Care Coordinator brings to you. Our Elder Care Coordinator will perform a personalized assessment of your care needs and will work to make sure the appropriate care is made available to you.

What Our Elder Care Coordinator Can Do for You

Expect unbiased, compassionate advice on care issues from our Elder Care Coordinator. You will receive assistance with:

  • Assessing care needs and living situations to identify problem areas and assist in solving them.
  • Coordinating among medical and healthcare providers.
  • Determining when outside care is needed.
  • Identification of care providers to client and family.
  • Educating clients and family on selecting and managing service providers, including residential facilities.
  • Arranging for necessary care services.
  • Advice regarding available payment sources for needed care including public benefit programs and private funding sources (such as Medicare, Medicaid, VA, Waiver, private insurance, long-term care insurance, health insurance, housing options).
  • Acting as an advocate on client’s behalf, and correcting problems with caregivers, including residential facilities, to maximize provision of high quality care.
  • Empowering you to effectively manage your loved one’s care.
  • Providing on-going support to client and family as client’s needs change, adjusting the Elder Care Plan as necessary.
  • Counsel to family on the selection of a nursing home (if and when necessary).
  • Educating family on how to supervise a client’s care.
  • Providing updated information through alerts and newsletters.
